Hello! Welcome to Thrive Home Care. We are that you are interested in applying to become a part of our team of dedicated and compassionate home care providers. Our application process will take approximately 60 minutes to complete. Below is your virtual checklist with links that will take you to the next step. Once you complete each step, come back to this page for the next link.
Step 1: Fill out our online application
Step 2: Take the Personal Care Assistant / Home Care Aide Exam
Step 3: Fill out the Personnel Record Form
Step 4: You will be given a Home Care Aide Registry Checklist by the HR Department. Please review and submit the necessary documents. You will have 7 days to complete, otherwise, your employment process will be affected. Click here to fill out the Application For Home Care Aide Registration form.
Step 5: You will be given a Fingerprinting form to complete. You will take this form with you the day you are getting fingerprinted. Once you’ve completed the fingerprinting process, please submit this form to Thrive Home Care for records. You can also submit this form by:

Step 7: Fill out the Criminal Record Statement Form
Step 8: Fill out the Form of Payment sheet provided by the HR department.
Step 9: Send us a photo of yourself with clear and plain background. This will be the photo used on your I.D. Badge issued by Thrive Home Care before your first shift.
